Lipoprotein(a) – the inherited risk factor
Lipoprotein(a), often abbreviated Lp(a), is a genetically determined part of your cholesterol profile. Roughly one in five people has elevated levels, and most are unaware because the value is not included in standard blood tests. Elevated Lp(a) is an independent risk factor for heart attack, stroke and aortic valve disease – regardless of how good your routine cholesterol numbers are.
Why Lipoprotein(a) should be measured
Lp(a) is barely affected by diet, exercise or statin therapy. That is why the ESC and EAS guidelines recommend that everyone measures Lp(a) at least once in their lifetime:
- The level is genetically determined and essentially constant throughout life
- One in five has elevated levels – often without knowing it
- The value is not detected by standard blood tests and requires a specific analysis
What do the studies show?
Research in recent years has established Lipoprotein(a) as a causal factor in cardiovascular disease:
- Mendelian randomization studies (NEJM, 2018) showed a causal link between high Lp(a) and heart attack as well as aortic valve disease
- The phase 3 trials Lp(a)HORIZON (pelacarsen) and OCEAN(a) (olpasiran) are evaluating whether lowering Lp(a) also reduces clinical risk
- Early phase 1/2 studies show the new drugs can lower Lp(a) by up to 90–95%
New medicines coming soon
Several new Lp(a)-lowering medicines are in development or nearing launch:
- Pelacarsen (antisense oligonucleotide) and olpasiran and lepodisiran (siRNA) – injections given weekly or monthly
- Muvalaplin – the first candidate that can be taken as a tablet
- Registration is expected within the next few years, depending on ongoing phase 3 trials and regulatory approvals
Why this matters for preventive care
Finding elevated Lp(a) early means we can set stricter LDL targets, follow you more closely and screen your relatives, since the level is inherited by half of your children. Today, elevated Lp(a) is managed indirectly by lowering all other cardiovascular risk. When the new medicines arrive, there will for the first time be a treatment that directly lowers this inherited risk factor – but that requires you to already know your value.
